The 6th Annual Health Forum is scheduled for February 28th - March 1st, 2012.

Registrations* are now being accepted and a draft agenda is available. We are pleased to be able to provide accommodations (as well as breakfast and lunch) during the forum for qualified registrants however due to limited funding, we are unable to provide travel reimbursements.

*Qualified registrants only. Qualifications listed below:

Must be a health manager or health worker (i.e. CHR) currently employed with a First Nation in Ontario.
First Nation Leadership (Ontario)
Designate

Please note, it is the responsibility of each registrant to communicate their participation with their respective community leadership/superiors before submitting a registration form. A maximum of 2 (two) delegates per First Nation are eligible.
